Musculoskeletal care for Basingstoke

Specialist care for shoulder pain

What is shoulder pain?

Shoulder pain will impact up to 1 in 4 UK adults in their lifetime.

This can be problematic as the shoulder is essential for lifting, carrying, reaching and rotating when doing activities of daily living. Impairment can lead to a loss of independence and frustrate us. There are a number of different causes for shoulder pain such as frozen shoulder, rotator cuff disorders, arthritis, a fracture or dislocation. Understanding the pathology at the shoulder will dictate the approach to the treatment. I believe in educating my clients in the pathology and treatment approach as it will empower them to remain complaint, reducing the risk of treatment failure.

Answering your questions

  • Rotator cuff related shoulder pain (RCRSP) is a term that encompasses a diverse range of shoulder conditions. These conditions include subacromial pain (impingement) syndrome, rotator cuff tendinopathy, subacromial bursitis, and symptomatic partial and full thickness rotator cuff tears.

Rotator cuff related shoulder pain is very common, and accounts for around 70% of all shoulder pain presentations. It can develop due to:

    • Age related changes in shoulder structures

    • Overload or repetitive movements

    • Trauma

Other lifestyle factors such as smoking and being overweight may increase the chances of developing rotator cuff related shoulder pain.

  • This is dependent on the nature of the neck pain and the severity of the condition. For example, a severe muscular strain within the neck typically takes 8-12 weeks to fully recover. Whilst a mild strain may only take 2-4 weeks. If your neck pain is related to a chronic degenerative condition such as spondylosis, the aim will be to manage the neck pain and symptoms long term rather than aiming for a full recovery. Other conditions such as a cervical herniated disc typically takes 6-12 weeks for most people to recover.

  • Frozen shoulder, also known as adhesive capsulitis, is a condition where your shoulder becomes painful, stiff, and difficult to move.

There is not always an apparent cause. Several factors increase the risk of people developing frozen shoulder:

    • Age – frozen shoulder commonly affects people aged 40 to 70

    • Diabetes – frozen shoulder is more common in people with diabetes

    • Thyroid disease – frozen shoulder is more common in people who have problems with their thyroid gland

    • Trauma – frozen shoulder may occur if you have a shoulder injury

    • Immobilisation – frozen shoulder may occur if you have not been able to move your arm like normal

For many people, frozen shoulder has 3 phases:

    • Freezing – the pain worsens, resulting in a loss of shoulder movement, lasting about 4 to 6 months.

    • Frozen – the pain settles down, but the shoulder remains stiff. This lasts for a further 4 to 6 months.

    • Thawing – the stiffness improves, and the shoulder’s function and mobility return. This can take 6 months up to 2 years.
